Counseling Application

Before we can begin the counseling process, you must complete a U. S. Small Business Administration "Request for Counseling" form.

This form asks some basic questions to help clarify your expectations. It also includes some confidential demographic information that is used internally by the U. S. Small Business Administration. Finally, it includes certain disclaimer information with which you must be willing to agree before the counseling can begin.

There are three methods by which you may request counseling: 

1. The preferred method to request counseling is online

Here's what you need to do:

Fill out the online form as completely as possible. In doing so, please note the following:

At the very bottom of the Request for Counseling there is space to further detail your needs. Whether or not you checked "other" on the drop-down above it on the form, make sure you tell us in this space the kind of business you want to start or are currently in - restaurant, dry wall contractor, management consultant, gift shop, etc. For example, if your aim is to write a business plan, telling us that it is for a nail salon will allow us to match you with a SCORE member with relevant experience. Do not leave this section blank or you will hold up the assignment process and may not get the best counselor for your needs.

If you choose online by clicking below, you will be taken to a screen that allows you to enter the Score national data bank. Do not be confused by their use of the term “register.” Clicking on the First Time Visitor side of the screen will indeed take you to the “Request for Counseling” online form. On the left of the screen is a place that allows you to enter your e-mail address. This is for you to return to an already submitted request to make changes or add to your text. Proceed to to right side of the screen to New Client: Register Here and click on it which will take you to the request form.

2. You may download the Request for Counseling form, print a copy, complete it and mail it to us.
Upon receipt, we will assign a counselor who will call you to set up a meeting. Please mail the completed form to:
